INCOMING KINDERGARTEN STUDENT REQUIREMENTS

In accordance with MA Department of Public Health mandates, all students entering Kindergarten must have the following BEFORE entry to school:

1) Physical Examination

2) Up-to-Date Immunizations

Your child's pediatrician has a comprehensive knowledge of the health status of your child, so please make an appointment to have him/her perform the school requirements. Immunization records of all incoming Kindergartner students are due PRIOR to entry to school. To enable the process of these records, please submit the paperwork no later than June 30th. Please plan accordingly, as school exclusion mandates are applied to non-compliant students.

You may fax the information directly to the nurse's office, submit by mail or hand deliver to your child's school.
